İşbu Açık Rıza Metni, 6698 sayılı Kişisel Verilerin Korunması Kanunu veri sorumlusu sıfatıyla, ULUHAN tarafından veri sahiplerinden temin edilen kişisel verilerin ve özel nitelikli kişisel verilerin, işlenmesinin usul ve esaslara ilişkin olarak veri sahiplerinin açık rızalarının temini amacı ile düzenlenmiştir.
Kişisel Verilerin Korunması Kanunu 5 ve 6. Maddesi kapsamında ULUHAN tarafından aşağıda belirtilen kişisel verilerimin;
Ad, Soyad, Doğum Tarihi, Doğum Yeri, TC Kimlik Numarası, Uyruk,
Adres Bilgileri,
Sürücü Belgesine ilişkin bilgiler,
Askerlik ve Seyahat Engel bilgisi,
Eğitim ve öğretim bilgileri,
İstihdam bilgileri(Özgeçmiş, Referans bilgileri),
Adli sicil bilgileri,
Sağlık bilgileri,
Mesleki. beceri ve diğer lisans ve izinlerle ilgili bilgiler,
E-posta ve bilgi sistemleri hizmetleri kullanım kayıtları,
SGK Kayıt Bilgileri(işe giriş olması halinde)
Avukatlık Büronuz prosedürleri gereğince muhafaza edilmesini;
Kişisel Verilerin Korunması Kanunu 5 ve 6. Maddesi kapsamında Aydınlatma Metninde ayrıntılı şekilde açıklanan meşru amaçlarınız ve bun yönelik amaçlar ile sınırlı olmak kaydı ile gerçekleştirebilmek için veri toplanacağını, işleneceğini ve paylaşılacağını,
Kişisel Verilerin Korunması Kanunu 8 ve 9. Maddelerinde belirtilen kişisel veri işleme şartları ve amaçları çerçevesinde Avukatlık Büronuz tarafından toplanan kişisel verilerin amaçlar dahilinde karşılıklı gizlilik taahhüdü yapılan insan kaynakları ofisleri veya bu işle ilgili kişiler ve kanunen yetkili kamu kurumları ile paylaşılabileceğini,
Veri sahibi olarak haklarım konusunda işbu metinde yer alan hususlarla alakalı olarak Kanun’un 11. Maddesi kapsamında eksiksiz bir şekilde bilgilendirildiğimi ve kişisel verilerimin ve özel nitelikli kişisel verilerimin, belirtilen meşru amaçlarla işlenmesini ve belirtilmiş olan kişilere aktarılmasını, bu hususta tarafıma gerekli aydınlatmanın yapıldığını, baskı, tehdit, hata ve hile gibi durumlar söz konusu olmadan bilincim açık bir şekilde okuduğumu ve anladığımı açık rızam dahilinde KABUL EDİYORUM.
This Explicit Consent Text has been prepared by ULUHAN, in its capacity as the data controller under the Personal Data Protection Law No. 6698, for the purpose of obtaining the explicit consent of data subjects regarding the procedures and principles for the processing of personal data and sensitive personal data obtained from data subjects.
Within the scope of Articles 5 and 6 of the Personal Data Protection Law, I hereby consent to the retention by ULUHAN, in accordance with the procedures of your Law Office, of my personal data listed below:

I acknowledge that, within the scope of Articles 5 and 6 of the Personal Data Protection Law, my personal data will be collected, processed, and shared solely for the legitimate purposes explained in detail in the Clarification Text and for purposes related thereto;
I acknowledge that, within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the Personal Data Protection Law, the personal data collected by your Law Office may be shared, within the scope of such purposes, with human resources offices or persons related to this process with whom mutual confidentiality commitments have been made, and with legally authorized public institutions;
As the data subject, I acknowledge that I have been fully informed regarding my rights under Article 11 of the Law in relation to the matters contained herein, and I hereby GIVE MY EXPLICIT CONSENT to the processing of my personal data and sensitive personal data for the specified legitimate purposes and to the transfer thereof to the specified persons.
